JOB TITLE: Registered Nurse (RN)/ Wound Care Certified DEPARTMENT: Nursing POSITION SUMMARY: The Registered Nurse is responsible for implementation and follow through of the nursing process. This includes systematic data collection, assessment, development of care plan, implementation of care plan and revision of plan. The Registered Nurse manages and supervises the care of residents through a well-defined care delivery system. He/She also supervises other nursing personnel to assure quality outcome of care. This position includes elements of wound care and infection control. QUALIFICATIONS: Must have current registration, as a Registered Nurse, in the State of Minnesota. Must have successfully completed an approved School of Nursing. Must be wound care certified. Must have successfully completed or seek certification in Cardiopulmonary Resuscitation and emergency care for the health care provider. Must possess the ability to communicate verbally and in writing when directing the care of residents. Must be able to perform essential functions of the position with or without reasonable accommodation. RESPONSIBILITY: To Facilitate the Comprehensive Care Planning Process and Apply Clinical Standards of Nursing Practice Systematically collects data about residents through interview of resident and family, observation, review of medical records.Assesses residents and applies knowledge to facilitate care planning. Applies nursing diagnosis as pertinent.Assists in the development of care plan with input from the interdisciplinary team.Implements care plan by assigning responsibility and communicate residents' needs to designated staff.Assessing the effectiveness of the resident plan of care ongoing and documenting goals and outcomes met or changed.Provides clear and accurate information and documentation of the above, continually. RESPONSIBILITY: Supervises and Manages LPN's and CNA's, Manages all Staff When in Charge of Building Articulates expectations and delegates appropriate job duties to co-workers.Follows up on directives given to co-workers and assures completion of duties by receiving report from departing nursing staff.Assesses knowledge and skill of supportive nursing personnel to assure safe practice of nursing staff.Confronts, corrects and explains unprofessional behavior to these staff.Prioritizes work assignments consistent with the number and levels of staff available that shift and acuity of residents' care.Knows and is able to implement and coordinate fire, tornado and disaster procedures.Take necessary steps to replace appropriate nursing staff to maintain proper staffing level. This may include working as the staff nurse. RESPONSIBILITY: Has Knowledge and Consistently follows Standards of Documentation and Written CommunicationCompletes charting in accordance with health information standards and facility policy.Data is systematically collected, is communicated through report and documented in medical record.Gathers and documents data on admissions and discharges.Facilitate all physician orders and completes procedures to assure accurate follow-up.Initiates, reports and documents nursing orders.Provides report to CNA's and other professional staff, verbally or written about daily resident care needs and/or significant changes in condition. RESPONSIBILITY: Complies with Pharmaceutical Policies of the Rehabilitation CenterAdministers medications accurately according to policy (including handwashing and 6 rights).Has minimal medication errors, and follows company policies for reporting and documenting such.Signs out medications and treatments appropriately.Completely and accurately obtains and transcribes Physician Orders for medication and treatments.Uses Proper procedure for label changes.Follows proper emergency kit procedures.Follows control substance policy to include count at the end of each shift.Documents PRN's according to policy.
